1. Draw a single line through the incorrect entry so it is legible.
2. Write "error" next to the lined-out documentation and include the date, time and initials of the person correcting the document
3. Write the correct documentation near the error or why it is lined-out (like the wrong patient)
4. Only the person who made the error can correct the error
5. Never completely cover the error, it is seen as "hiding" something
6. Never use white-out or liquid paper.
